Our patient coordinators can provide new patient paperwork ahead of time, or please plan to arrive 15 minutes prior to your first appointment to complete necessary paperwork. In the near future, we will be offering easy online scheduling through our new patient portal!
The initial appointment is intended to determine what your needs are. Please allow 30-45 minutes for this initial meeting. It is our hope this process leaves you feeling comfortable and hopeful regarding a care plan made just for you based on a dialogue which allows you to express your concerns, interests and circumstances.

See below for the at a glance list but please call us with your insurance information, or simply bring your insurance card on your first visit, and we would be happy to assist you in determining what benefits you have in the office. We are providers for The Department of Labor and Industries, which allows us to see individuals who have injuries that are related to the workplace. We are on the provider list able to assist people who have suffered injury from violent crime. Anyone who has suffered an injury from a violent crime is entitled to Washington State benefits for their treatment.
